Is the patient alert and ready to sign?
Ensure the individual is mentally clear and able to willingly execute the documents when the Notary arrives.

Have you explained the notarization to the patient in advance?
Reach out to the patient before the Notary’s visit to the care facility to clarify why you’re arranging notarization and what each document covers. Remember, Notaries do not review legal content or provide advice—if the patient doesn’t understand what they’re signing, the Notary cannot proceed.

Can facility staff serve as a witness?
Many care centers prohibit their employees from acting as witnesses. Ask the facility in advance whether staff may witness your signing. If not, request that the Notary bring an approved witness—note that this may incur an extra fee.

Are your documents pre‑prepared and finalized?
Notaries cannot draft or prepare legal forms—such as advance healthcare directives, affidavits, or wills—unless they hold document‑preparer credentials or are attorneys. Always have your paperwork fully completed before the appointment.

How will you get your notarized documents back?
If you cannot attend the signing in person, discuss return options with your Notary—UPS, FedEx, or standard mail. Additional shipping or handling charges may apply.
